California Special Focus Facilities
The CMS Special Focus Facility (SFF) program targets U.S. nursing homes with the most serious and persistent quality problems. Facilities remain on the list for 18–24 months and must show substantial improvement or face termination from Medicare and Medicaid. Candidates are facilities under active consideration for SFF designation.
