About Starfile
Starfile indexes the U.S. Centers for Medicare & Medicaid Services Nursing Home Care Compare dataset and makes it browsable at the facility, deficiency, and ownership level. Every page on this site is generated from public-domain government data.
What we publish
- Facility pages for every Medicare/Medicaid-certified nursing home in the United States.
- Each inspection citation with its full surveyor narrative, scope & severity code, and correction status.
- Staffing ratios (hours per resident per day) alongside state and national averages.
- Fines, payment denials, and penalty counts on a rolling three-year window.
- Ownership chains, allowing a reader to see every facility sharing a common owner.
Data source & refresh
All data comes from data.cms.gov/provider-data/topics/nursing-homes. CMS refreshes the underlying files on the last Wednesday of each month. Starfile re-ingests within 48 hours of each CMS refresh; the processing date is shown on every facility page.
What this site is not
Starfile is an independent research tool. It is not affiliated with CMS, Medicare, the Office of the Inspector General, any nursing home, any nursing home chain, or any law firm. Nothing on this site is medical or legal advice. For a placement decision, please speak with a licensed advisor. Corrections
If you see a factual error, check it against the CMS file first — we mirror the data exactly. If the CMS record itself is wrong, the correction has to start there; CMS accepts error reports through the facility's state survey agency.
